Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality, patient-centered care across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, medication administration, and thorough assessments. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately, including IV infusions and catheterizations, following strict protocols and safety standards. Manage complex care needs such as dementia care, Alzheimer's care, memory care, hospice support, and acute pain management for diverse patient groups. Utilize electronic health record (EHR) systems like Epic or Cerner to document medical records meticulously and ensure compliance with HIPAA regulations. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs. Provide patient education on medication use, treatment procedures, and health maintenance strategies to promote self-care and recovery. Assist with diagnostic evaluations such as specimen collection/processing and medical imaging procedures when necessary. Experience Valid licensure as a Licensed Practical Nurse (LPN) in accordance with state regulations. Proven experience working with geriatric patients in settings such as skilled nursing facilities or long-term care environments. Demonstrated proficiency in medical documentation using EMR/EHR systems like Epic or Athenahealth. Critical care experience preferred, including ICU or Level I/II Trauma Center exposure. Familiarity with specialized areas such as hospice care, infectious disease management, trauma response, or pediatric nursing is highly desirable. Experience working with individuals with disabilities or developmental challenges is a plus. Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and infection control protocols is essential for success in this role.
