The ideal candidate will possess a strong foundation in patient care, with experience across various medical settings, including short term and long-term care. As an LPN, you will play a vital role in providing high-quality nursing services, supporting patient recovery, and ensuring compliance with healthcare regulations. Your expertise will contribute to creating a safe, respectful, and nurturing environment for our patients. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using electronic health record (EHR) systems. Administer medications and injections accurately while adhering to HIPAA and sterile processing protocols. Assist with diagnostic evaluations such as specimen collection, laboratory processing, and medical imaging procedures. Provide wound care, catheterization, IV infusion, and tube feeding support in accordance with established clinical guidelines. Monitor patients' conditions closely, manage acute pain, and respond promptly to emergencies. Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans. Maintain meticulous medical documentation ensuring compliance. Support discharge planning, patient education, and caregiver communication to promote ongoing health management at home or in assisted living environments. Qualifications Valid licensure as a Licensed Practical Nurse (LPN). Proven experience working in multiple healthcare settings such as hospitals, nursing homes, hospice care, or outpatient clinics. Familiarity with EMR/EHR systems. Experience with specialized care areas including Alzheimer's/dementia care, memory care, infectious disease management. Strong clinical skills in patient assessment, medication administration, sterile technique, and first aid procedures. Excellent communication skills to effectively coordinate with patients' families and multidisciplinary teams while maintaining professional confidentiality.
