Job Description

LPN:
Licensed Practical Nurse Skilled Nursing | Three Pillars Senior Living Communities | Dousman, WI Full Time | Day Shifts Part Time | Day & Evening Shifts Pool | Minimum two shifts per month
Full Time & Part Time:
$32-$37.25/hour based on experience & shift differentials
Per Diem:
$27-$38/hour based on experience & pick up pay ( $5/hour weekdays and $7/hour weekends) Make a difference with Three Pillars Senior Living Communities, located in Dousman, WI! LPN:
Licensed Practical Nurse Qualifications:
Active Wisconsin Licensed Practical Nurse (LPN) license Current CPR certification Long-term care experience preferred Basic computer skills Ability to push/pull up to 60 lbs. and occasionally lift up to 100 lbs.
What You'll Do:
Safely provide medications and treatments as prescribed by physicians, following accepted standards of practice and organizational policies. Maintain accurate, timely written and electronic records in compliance with legal, regulatory, and company requirements. Monitor and document residents' conditions, report changes to the RN and relevant team members, and collaborate in developing and implementing individualized care plans. Assist with admissions and readmissions, gather clinical data, support resident care programs by ensuring adherence to plans of care, and oversee CNA performance in these areas.
