Overview Join our dynamic healthcare team as a Certified Family Nurse Practitioner (FNP), where you will play a vital role in delivering comprehensive, patient-centered care across diverse settings. As an FNP, you will provide primary and specialized care, conduct assessments, develop treatment plans, and prescribe medications within your scope of practice. Having a patient following is highly reccomened. Responsibilities Perform thorough physical examinations and health assessments for patients of all ages, including pediatrics and geriatrics. Develop and implement individualized care plans based on diagnostic evaluations, patient history, and clinical findings. Prescribe medications and treatments in accordance with state regulations and collaborative practice agreements. Conduct procedures such as suturing, IV infusions, catheterizations, and dermal filler injections when appropriate. Utilize electronic health record (EHR) systems like Epic, eClinicalWorks, or Cerner to document patient encounters accurately and efficiently. Collaborate closely with physicians, specialists, nurses, and other healthcare professionals to coordinate comprehensive patient care. Engage in telehealth consultations to extend access to quality healthcare services remotely. Manage acute pain cases, perform triage assessments, and participate in hospital or emergency room settings when needed. Contribute to clinical research initiatives or quality improvement projects aimed at enhancing patient outcomes. Educate patients on health promotion, disease prevention, medication management, and lifestyle modifications. Requirements Valid certification as a Family Nurse Practitioner from an accredited program; current state licensure required. Experience working with diverse populations including children, seniors, individuals with disabilities or chronic conditions such as dementia or Alzheimer's disease. Proficiency with EMR/EHR systems such as Epic, Athenahealth, or Cerner; experience with medical documentation standards including
ICD-10/ICD-9
coding essential. Strong clinical skills in areas such as acute care, critical care (ICU/Level I & II trauma centers), emergency medicine, geriatrics, behavioral health, and hospice & palliative medicine preferred. Ability to perform procedures like suturing, phlebotomy, venipuncture, spinal taps, gastrostomy tube management, and sterile techniques. Knowledge of pharmacology for prescribing medications within DEA regulations; experience with drug interactions and medication reconciliation. Excellent client communication skills; ability to educate patients effectively on treatment plans and health coaching strategies. Critical thinking skills with experience in triage protocols, diagnostic evaluation, case management or utilization review highly desirable. Prior experience working in hospital medicine settings such as NICU/PICU or trauma centers is advantageous; familiarity with hospital policies including HIPAA compliance is required.
