Job Overview We are seeking a compassionate and dedicated Hospice Nurse Practitioner to join our team. In this role, you will provide comprehensive face to face visits for patients with life-limiting illnesses, focusing on enhancing their quality of life and supporting their families. You will work collaboratively with a multidisciplinary team to develop and implement patient-centered care plans, ensuring that all aspects of patient needs are addressed. Duties Conduct thorough assessments of patients' physical, emotional, and psychosocial needs. Provide education and support to patients and families regarding disease processes, treatment options, and end-of-life care. Manage symptoms effectively through appropriate interventions, including psychopharmacology when necessary. Maintain accurate documentation of patient assessments, interventions, and outcomes in compliance with regulatory standards. Requirements Current Nurse Practitioner license with certification in hospice or palliative care preferred. Certification in Basic Life Support (BLS) is required. Excellent communication skills to effectively interact with patients, families, and healthcare professionals. Ability to work collaboratively within a multidisciplinary team environment. $70.00 - $72.66 per hour
