NP - Geriatric Medicine Job in Brooklyn, New York

Job Description

We are seeking an experienced Nurse Practitioner or Physician Assistant for a locum-to-perm opportunity (conversion expected within 3-6 months) at a value-based primary care practice serving geriatric patients in New York City. This hybrid role combines outpatient clinic hours with home-based visits. Compensation $75-$95 per hour Schedule & Clinical Load Full-time, guaranteed hours Monday-Friday, five days per week; typical start at 8:30
AM Clinic:
small panel, roughly 7-10 patients/day Home visits: approximately 3-5 patients/day
Setting:
hybrid (outpatient clinic + home-based care) focused on value-based care for older adults Key Responsibilities Provide comprehensive primary care for geriatric patients, including preventive care, chronic disease management, and acute issue treatment Diagnose, treat, prescribe, and develop individualized care plans Order and interpret diagnostic tests (labs, imaging) and manage follow-up care Accurately document clinical encounters in the EHR Educate patients and caregivers; promote wellness and care coordination Collaborate with physicians, nurses, and support staff to ensure high-quality, coordinated care Make referrals to specialists and track outcomes Maintain compliance with state, federal, and professional regulations Requirements Active NP or PA license in New York State Minimum 2 years' primary care experience (geriatric experience preferred) National board certification Ability to obtain expedited credentialing Medicaid and Medicare provider ID required This role is ideal for clinicians who enjoy longitudinal, value-based care for older adults and a mixed clinic/home visit schedule.
