Job Description

Provide high-quality, patient-centered care to residents in skilled nursing facilities across Brooklyn, NY, within a collaborative, team-based environment. Openings also available in IL, MI, NH, NC, OH, and WI. Schedule and duties Full-time with flexible scheduling Six-hour facility shifts for full-time providers (rounds typically start between 7:00-10:00 AM) No call or overnight responsibilities Perform daily rounds, manage acute and chronic conditions, prescribe medications and treatment plans Document efficiently using streamlined EHR with AI scribe, voice dictation, and company-provided iPad with LTE Access UpToDate® for clinical guidance Compensation and benefits Competitive base pay with uncapped productivity-based compensation potential Up to $50,000 in long-term retention incentives Performance bonuses Paid time off 401(k) with employer match Medical, dental, vision, life, and disability insurance (eligibility applies) CME allowance and licensure reimbursement Company-provided iPad with LTE and lab coat Sign-on bonus and relocation assistance for eligible candidates Requirements Active, unrestricted Nurse Practitioner or Physician Assistant license Relevant board certification (ANCC, AANPCB, PNCB, AACN, NCCPA as applicable) Experience in skilled nursing, long-term care, geriatrics, or post-acute care preferred
