No call responsibilities, weekend coverage, or holiday shifts are required. HouseCalls is a unique, outcomes-focused program that delivers healthcare directly in members' homes. As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you'll perform annual in-home health assessments for Medicare Advantage and other eligible health plan members. This field-based, non-prescribing role is designed to support preventive care, close care gaps, and collaborate with primary care providers to improve overall health outcomes.

Primary Responsibilities:
Complete comprehensive in-home health evaluations, including:

Medical history review and medication reconciliation

Vital signs assessment and physical examination

Evidence-based screenings and point-of-care testing when appropriate

Identify and document conditions that support care planning and disease management

Communicate assessment findings to primary care providers to help address care gaps

Recognize and appropriately respond to urgent or emergent health concerns

Educate members on chronic conditions, medications, and treatment adherence

Assess social determinants of health and connect members with appropriate resources and services
