Responsibilities Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility to promote comfort and independence. Monitor and record vital signs such as blood pressure, temperature, pulse, and respiration rates accurately using electronic health record (EHR) systems. Support medication administration under supervision, ensuring proper documentation in compliance with HIPAA regulations and medical documentation standards. Provide emotional support and companionship to patients and their families, respecting cultural sensitivities and individual preferences. Collaborate with the healthcare team to develop and implement personalized care plans tailored to each patient's needs. Utilize safe manual handling techniques and equipment like Hoyer lifts to assist with patient transfers safely. Maintain patient confidentiality at all times while managing medical records with precision using EMR/EHR systems. Requirements Current certification as a Certified Nursing Assistant (CNA) with valid state licensure manadatory. Experience working in hospice or palliative care settings is highly preferred; familiarity with end-of-life protocols is essential. Demonstrated ability to work compassionately with individuals facing terminal illnesses, including those with dementia or Alzheimer's disease. Knowledge of medical terminology, vital signs monitoring, medication administration, and aseptic techniques. Strong communication skills for effective caregiver-patient interaction and interdisciplinary collaboration. Ability to perform heavy lifting safely using proper techniques and equipment such as Hoyer lifts when necessary. Prior experience in long-term care facilities, hospitals (including ICU or trauma centers), or home & community care environments is advantageous; familiarity with EMR/EHR systems like Cerner or eClinicalWorks is a plus.
