RN Care Manager (Home Health)
Job Summary:
We are seeking a compassionate and dedicated RN Care Manager to provide skilled nursing care to patients in their homes. The RN Care Manager is responsible for managing an assigned patient caseload, conducting comprehensive patient assessments, developing and implementing individualized care plans, coordinating care with interdisciplinary teams, educating patients and caregivers, and ensuring safe, high-quality home health services.
Requirements:
Active Registered Nurse (RN) license in the state of practice Current BLS/CPR Certification Valid Driver's License Reliable transportation and current automobile insurance BSN preferred Acute Care or Home Health experience preferred Strong assessment, communication, documentation, and care coordination skills
Key Responsibilities:
Conduct initial and ongoing comprehensive patient assessments Develop, implement, and update individualized plans of care Manage an assigned patient caseload efficiently Coordinate patient care with physicians and interdisciplinary healthcare teams Provide skilled nursing care in the patient's home Educate patients and caregivers regarding treatment plans and disease management Maintain accurate and timely clinical documentation Monitor patient progress and modify care plans as needed Ensure compliance with HIPAA, agency policies, and regulatory requirements Participate in on-call rotation as assigned Perform other job-related duties as assigned !
