Job Description

Job Title:
Certified Occupational Therapy Assistant (COTA) |
Home Health Company:
Legacy Home Care / Home Health Care, Inc.
Location:
Mankato, Waseca, St.
Peter, Fairmont, Blue Earth, Le Sueur & Surrounding Communities Schedule:
Full-Time or Part-Time |
Monday-Friday Compensation:
$82,000-$110,000 per year Want to stand out from other applicants? Email Nikky Vogelgesang at nikky.vogelgesang@hhcare.net with your resume and tell us about your COTA background, what you're doing today, and what you enjoy most about helping patients regain independence. Sometimes the Difference Between Staying Home and Needing More Help Is Occupational Therapy. Can they get dressed safely? Can they get themselves to the bathroom? Can they prepare something to eat? Can they safely shower? Can they get through their morning routine without relying entirely on someone else? These aren't small questions. They can determine whether someone is able to remain independent in the place they call home. Legacy Home Care and Home Health Care, Inc. are hiring a Certified Occupational Therapy Assistant (COTA) to provide home-based therapy throughout Mankato, Waseca, St. Peter, Fairmont, Blue Earth, Le Sueur, and surrounding Southern Minnesota communities . This is an opportunity to take the COTA skills you've already built and use them where functional independence matters most. Help Patients Get Back to Doing, Not Just Recovering A patient's strength may be improving. Their incision may be healing. Their medical condition may be stabilizing. But can they actually manage their day? That's where Occupational Therapy becomes incredibly important. Working under the direction of an OTR/L, you'll help patients turn clinical progress into real-world independence . Getting dressed. Preparing a meal. Safely navigating the bathroom. Using adaptive equipment. Developing a safer way to complete a task they've done the same way for decades. The goal isn't simply improvement. It's helping someone do more for themselves. Why Consider This Opportunity? $82,000-$110,000 annual earning potential Monday-Friday schedule Full-Time and Part-Time opportunities One-on-one patient visits Start your day from home Territory-based scheduling Consistent patient volume Mileage reimbursement between visits Cell phone reimbursement each paycheck Free Education Hub access for continuing education Licensure reimbursement $250 annual scrub reimbursement Medical and dental insurance Life and disability coverage PTO and sick time 401(k) with employer match Experienced OTR/L and interdisciplinary support Treat the Patient Where Everyday Life Actually Happens Home health gives Occupational Therapy a unique advantage. You aren't recreating the patient's bathroom in a therapy gym. You're standing in their bathroom. You can see the height of the bed. The layout of the kitchen. The chair they struggle to stand from. The narrow hallway. The shower they're afraid to use. That gives you the opportunity to help patients develop strategies that work for their actual life and environment . What You'll Do Provide Occupational Therapy treatments in patient homes under the direction of an OTR/L Carry out individualized treatment plans Improve ADLs and functional independence Address strength, coordination, mobility, and functional skills Reinforce adaptive and compensatory strategies Provide adaptive equipment and home-safety education Educate patients, families, and caregivers Monitor response to treatment and communicate progress to the supervising OT Complete accurate and timely EMR documentation Collaborate with the interdisciplinary care team Your COTA Experience Can Translate Home health experience is valuable, but it isn't required . We're interested in COTAs from Skilled Nursing, Inpatient Rehabilitation, Outpatient Therapy, Transitional Care, Senior Living, Long-Term Care, Hospital-Based Rehabilitation, Geriatrics, and Home Health . You'll need an active Minnesota COTA license , valid driver's license and reliable transportation, strong communication and organizational skills, and the confidence to provide care independently in patient homes while collaborating closely with your supervising OTR/L. Help Someone Keep Doing the Things That Make Home Feel Like Home Occupational Therapy can be the difference between someone needing help with nearly everything and being able to safely do more for themselves. That's meaningful anywhere. But there's something different about helping someone regain that independence inside their own home. If you're looking for a COTA opportunity with $82,000-$110,000 earning potential, Full-Time or Part-Time options, Monday-Friday scheduling, one-on-one care, and meaningful functional therapy , we'd like to meet you. Want to stand out from other applicants? Email Nikky Vogelgesang at nikky.vogelgesang@hhcare.net with your resume and tell us about your COTA background, what you're doing today, and what you enjoy most about helping patients regain independence.
