Legacy Home Care and Home Health Care, Inc. are hiring a Certified Occupational Therapy Assistant (COTA) to provide home-based therapy throughout Mankato, Waseca, St. Peter, Fairmont, Blue Earth, Le Sueur, and surrounding Southern Minnesota communities . What You'll Do Provide Occupational Therapy treatments in patient homes under the direction of an OTR/L Carry out individualized treatment plans Improve ADLs and functional independence Address strength, coordination, mobility, and functional skills Reinforce adaptive and compensatory strategies Provide adaptive equipment and home-safety education Educate patients, families, and caregivers Monitor response to treatment and communicate progress to the supervising OT Complete accurate and timely EMR documentation Collaborate with the interdisciplinary care team Your COTA Experience Can Translate Home health experience is valuable, but it isn't required . We're interested in COTAs from Skilled Nursing, Inpatient Rehabilitation, Outpatient Therapy, Transitional Care, Senior Living, Long-Term Care, Hospital-Based Rehabilitation, Geriatrics, and Home Health . You'll need an active Minnesota COTA license , valid driver's license and reliable transportation, strong communication and organizational skills, and the confidence to provide care independently in patient homes while collaborating closely with your supervising OTR/L.
