This pharmacy operates under a "Specialty-Plus" Model, dispensing both Specialty and Mon-Specialty Medications pertinent to specific disease states that often require a higher level of care. This frequently involves a multidisciplinary, collaborative approach to ensure proper medication coordination and administration. Patient populations currently served include Hemophilia and Cystic Fibrosis. Participate on committees and give special reports. Rotate through the distributive pharmacy as scheduled. Directly perform order entry in the pharmacy; Provide all of the functions as a staff pharmacist; Perform other duties as assigned. Provide patient and community education; Participate in the staff development education program; Provide clinical training to Pharm. D. students as assigned. Perform accurate and timely input of interventions and ADR's. Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Adhere to the organization's published values Qualifications Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy 2 years of relevant experience American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred LIC-Clinical Pharmacist
