- Masters Degree in Physical Therapy or Bachelor's degree in Physical Therapy with a minimum 5 years of work experience. Licenses and Certification
- Illinois license in Physical Therapy
- BLS Experience and Skills
Technical Experience:
N/A Role Specific Responsibilities- Performs thorough evaluation
- Develops appropriate Plan of Care
- Implements Plan of Care
- Documentation is accurate, complete and timely
- Provides supervision to assistants, rehab techs, and students Compensation (Commensurate with experience): $41.
