- Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part. Education
- Masters Degree in Physical Therapy or Bachelor's degree in Physical Therapy with a minimum 5 years of work experience. Licenses and Certification
- Illinois license in Physical Therapy
- BLS Experience and Skills
