Pride Health is hiring a Physical Therapist (PT) - Home Health to support our client's Home Health Team in

NORTH EASTON, MA.

Dedham, Braintree, Quincy, and surrounding communities.

Pay Range:

$50.00 to $57.00/hour

Mileage Reimbursement Rate (if applicable): $0.52/mile

•Pay offered is based on experience, expertise, credentialing, and education.

Duties:

Perform comprehensive physical therapy evaluations and assessments in the home setting.

Develop individualized Plans of Care (POC) based on physician orders, patient needs, and clinical findings.

Deliver evidence-based therapeutic interventions to improve strength, mobility, balance, endurance, and functional independence.

Conduct reassessments and modify treatment plans as needed to achieve optimal patient outcomes.

Supervise and mentor Physical Therapist Assistants (PTAs), ensuring treatment plans are implemented appropriately and in compliance with regulatory standards.

Collaborate with physicians, nurses, occupational therapists, speech therapists, caregivers, and interdisciplinary team members to coordinate patient care.

Educate patients and families on therapeutic exercises, fall prevention, home safety, mobility training, and disease management strategies.

Complete accurate and timely clinical documentation while maintaining productivity expectations.

Requirements:
  • Active Massachusetts Physical Therapist (PT) License in good standing.
  • Graduate of an accredited Physical Therapy program.
  • Previous Home Health experience required.
