Job Description
Atlantic Health is seeking a dynamic and motivated individual, to join the energetic and innovative team of healthcare professionals in their Atlantic Visiting Nurse Division as a Part Time Physical Therapist in Morris County. The ideal candidate will evaluate and treat patients in the patient's home and be a part of a collaborative team of health providers. Schedule is flexible between Monday through Friday and 8-4pm daily. Also, weekend rotation every 5 to 7 weeks. Atlantic Visiting Nurse provides personalized care for you or a loved one to regain health, stay out of the hospital, and remain living independently at home. Whether or not it is managing life with a chronic illness, living with a disability, returning home from the hospital or rehabilitation facility, or need assistance with end-of-life care, we are available to you in six counties throughout
New Jersey:
Morris, Somerset, Essex, Sussex, Union, and Warren.
Principal Accountabilities:
1. Performs initial examinations and differential diagnosis. 2. Provides and prescribes plans of care consisting of corrective therapeutic exercise. 3. Performs manual treatments consisting of soft tissue techniques, joint mobilizations, and many more. 4. Supervises and interacts with support staff and maintain proper documentation. 5. Evaluates and establishes a plan of care and treats patients accordingly. 6. Documents patient care and charges appropriately, and documents the course of patient care, including progress made and the ongoing need for treatment. 7. Works with other professionals in an effective manner 8. Provides clinically appropriate therapeutic interventions. 9. Evaluates patient response to treatment and provides feedback to the interdisciplinary team. 10. Performs other related duties as assigned.
Minimum Education:
High School Diploma or GED, required Bachelor's in Physical Therapy from an accredited college or university, required Master or Doctorate in Physical Therapy preferred
Minimum Experience:
1 year of experience in homecare, strongly preferred
License/Certification:
NJ License in Physical Therapy BLS required Valid and unrestricted Driver License with a clean driving record, required.
