Occupational Therapist/Physical Therapist - SNF (1099 Positions Available)

Job Description

Job Summary:
We are seeking skilled and compassionate PRN Occupational & Physical Therapists to join our dynamic team at Cliffside Rehabilitation & Health Care Center. With our team, you will play a vital role in improving the quality of life for our residents by providing comprehensive Therapy services. You will work closely with interdisciplinary teams to develop and implement personalized treatment plans to enhance independence, promote functional abilities, and maximize overall well-being.
Job Details:
Competitive salary commensurate with experience and qualifications. Per diem work - coverage, evenings, weekends 1099
Positions Available Responsibilities:
Conduct thorough evaluations of residents' physical, cognitive, and psychosocial abilities to determine individualized treatment goals and plans. Design and implement therapy interventions to improve residents' independence, functional skills, and activities of daily living. Provide interventions such as therapeutic activities/exercises, manual therapy, adaptive equipment training, and environmental modifications to enhance residents' physical and cognitive abilities and restore function. Collaborate with interdisciplinary teams, including physicians, nurses, speech therapists, and physical therapists/assistants, to ensure comprehensive and coordinated care. Document all evaluations, treatments, and progress notes accurately and in a timely manner. Adhere to ethical and professional standards as set forth by relevant regulatory bodies and professional associations (HIPAA compliance).
Qualifications:
A valid license to practice as an Occupational/Physical Therapist in the state. Graduation from an accredited Occupational/Physical Therapy program. Prior experience in a skilled nursing facility or geriatric care setting is required. Strong knowledge of occupational /physical therapy principles, practices, and techniques. Proficiency in using relevant therapy documentation software (Net health, Sigma Care)
Pay:
$60.00 per hour
