Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this role, you will be responsible for assessing, developing, and implementing personalized treatment plans to help patients regain mobility, reduce pain, and improve their overall quality of life. You will work closely with a multidisciplinary team to deliver exceptional patient care across various settings, including outpatient clinics, inpatient hospitals, home health environments, and rehabilitation centers. Your expertise in physiotherapy and patient-centered care will be vital in promoting recovery and enhancing functional independence for diverse patient populations. Responsibilities Conduct thorough evaluations of patients' physical conditions, including mobility, strength, flexibility, and pain levels. Develop comprehensive treatment plans tailored to individual needs, incorporating evidence-based practices such as trigger point therapy and orthopedics. Implement therapeutic interventions aimed at improving patient outcomes, including manual therapy, exercise programs, and functional training. Collaborate effectively with healthcare professionals such as physicians, nurses, occupational therapists, and social workers to coordinate care strategies. Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR) or electronic medical records (EMR) systems. Assist in discharge planning by evaluating patient progress and recommending appropriate home & community care or outpatient rehabilitation services. Educate patients and their families on treatment goals, physiotherapy techniques, injury prevention, and self-care strategies to promote ongoing recovery. Qualifications Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within acute care, inpatient hospital settings, outpatient clinics, or home health environments. Strong knowledge of physiology, anatomy, neurology, orthopedics, geriatrics, pediatrics, and related medical terminology. Demonstrated ability to develop effective treatment plans based on clinical assessments and patient needs. Experience working with geriatric patients and providing outpatient rehabilitation services. Proficiency in managing electronic health records (EHR) or electronic medical records (EMR) systems with attention to detail for documentation standards. Excellent client communication skills with the ability to motivate and educate diverse patient populations. Familiarity with discharge planning processes and working collaboratively with healthcare teams to ensure seamless patient transitions. Knowledge of inpatient and outpatient care protocols along with home & community care experience is highly desirable. $60.00 - $80.00 per hour
