Job DescriptionXcelerate Physical Therapy is looking for a motivated individual to join our team as a Physical Therapist. What are some of the things you will do:

Assess patients to develop and initiate treatment plans based on the evaluation resultsReview the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment requiredProvide direct patient care by established protocols and patient care guidelinesDocument findings, progress, and instructions to patients and caregiversResponsible for patient care performed by assistants and rehabilitation techniciansComplete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goalsImplement physical therapy treatment program and provide essential physical therapy treatmentsCommunicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companiesEnsure timely documentation, including daily notes, progress reports, initial evaluations, discharge summaries, and incident reportsQualificationsGraduate from a CAPTE accredited Physical Therapy Program New or recent graduates are encouraged to apply Current state of PA licensure as Physical Therapist (PT)CPR certification Ability to work efficiently/effectively in an autonomous environment Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office Additional InformationCompetitive pay401KMultiple opportunities for professional development, specialization, and leadershipMentoring and onboardingCorporate discount plansEmployee Assistance Program (EAP)Family-friendly work environmentInvestment from a company that wants you to succeed and thrive
