Job Description
South Pacific Rehab Services is seeking a compassionate Physical Therapist Assistant to provide resident-centered rehab services in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with therapeutic exercises, mobility, balance and gait training, and functional activities to promote safety and independence. You'll document treatments, collaborate with an interdisciplinary team, educate patients and caregivers, and support outcome-focused programs in a clinician-centered, ethical, and growth-oriented environment. Responsibilities Provide physical therapy interventions under direction of a licensed Physical Therapist Implement treatment plans focused on mobility, strength, balance, and functional independence Assist with gait training, transfer training, and therapeutic exercises Monitor patient progress and communicate changes to the supervising PT and care team Accurately document treatments, patient response, and progress in the medical record Educate patients, families, and caregivers on exercises, safety, and functional strategies Collaborate with nursing, physicians, and interdisciplinary team members on care plans Support discharge planning and recommendations for equipment and continued services Adhere to all regulatory, ethical, and company clinical standards and protocols Participate in ongoing education, in-services, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training (transfers, bed mobility) Use of assistive devices and DMEVital signs monitoring and patient safety Rehab documentation and EMR usage Knowledge of post-acute and skilled nursing rehab regulations Patient and family education techniques Body mechanics and safe patient handling Interdisciplinary team communication Range of motion and strengthening techniques Knowledge of treatment modalities (e.g., heat, cold, e-stim per state rules)
