Renewal Rehab is seeking a Certified Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. In this role, you will deliver compassionate, evidence-based interventions under the direction of a Physical Therapist to improve mobility, function, and quality of life. You will treat manageable caseloads, document in an EMR, and collaborate with an interdisciplinary team. Responsibilities Deliver physical therapy interventions under the direction of a licensed Physical Therapist for seniors in skilled nursing and post-acute care. Implement individualized treatment plans focusing on functional mobility, strength, balance, and safety. Accurately document treatments, progress notes, and outcomes in the EMR in a timely, compliant manner. Educate patients, families, and caregivers on home exercise programs and safe mobility strategies. Collaborate with an interdisciplinary care team to support patient-centered goals and discharge planning. Monitor patient response, report changes to the PT, and recommend adjustments to the plan of care.
