Responsibilities Deliver physical therapy treatments under the supervision of a Physical Therapist for seniors in skilled nursing and post-acute settings. Follow individualized care plans to improve mobility, strength, balance, and functional independence. Accurately document treatments, patient progress, and outcomes in accordance with facility and regulatory standards. Educate patients, families, and caregivers on exercises, safety, and use of assistive devices. Collaborate with PTs, nurses, and the interdisciplinary team to support patient-centered, goal-driven care. Ensure compliance with ethical standards, safety protocols, and organizational policies. Participate in mentorship, in-services, and professional development opportunities offered by Renewal Rehab.
