Job Description

Renewal Rehab seeks a compassionate Physical Therapist Assistant to provide high-quality, evidence-based therapy services for seniors in skilled nursing and post-acute care settings. In this collaborative, patient-centered role, you will implement PT treatment plans to improve mobility, strength, balance, and functional independence while documenting progress and outcomes. Responsibilities Deliver physical therapy treatments under the supervision of a Physical Therapist for seniors in skilled nursing and post-acute settings. Follow individualized care plans to improve mobility, strength, balance, and functional independence. Accurately document treatments, patient progress, and outcomes in accordance with facility and regulatory standards. Educate patients, families, and caregivers on exercises, safety, and use of assistive devices. Collaborate with PTs, nurses, and the interdisciplinary team to support patient-centered, goal-driven care. Ensure compliance with ethical standards, safety protocols, and organizational policies. Participate in mentorship, in-services, and professional development opportunities offered by Renewal Rehab. Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training PT plan of care implementation Patient and family education Documentation in EMR systems Range of motion and strengthening techniques Safety and fall-prevention practices Interdisciplinary team collaboration Knowledge of geriatric conditions and precautions
