SKILLED NURSING & OUTPATIENT THERAPY!
As a PTA, you'll be an integral part of our continuum of care, providing SNF therapy as well as 1-on-1 outpatient therapy to patients from outside the SNF. Why choose the hybrid SNF/OP role? This role is ideal for both new grads seeking experience in multiple settings and seasoned therapists looking to treat a variety of diagnoses. In the SNF, you'll provide 1-on-1, concurrent, and group therapy as appropriate. In outpatient, you'll provide 1-on-1 sessions for 45-60 minutes in our beautifully updated gym at Woodward Hills. The role generally offers a consistent outpatient schedule that flexes with SNF caseloads, helping ensure consistent hours. Rivetus has served the geriatric Post-Acute community since 1998. We are a specialized rehabilitation company operated and managed at the executive level by licensed clinicians, providing continuity of care across Skilled Nursing, Home Health, and Outpatient settings.

Facility Location:
38200 Schoenherr Road, Sterling Heights, MI 48312 •This is a hybrid role providing outpatient therapy in conjunction with SNF-based therapy services. Outpatient and SNF services are paid at an hourly rate.
General Summary:
Provides physical therapy treatment according to established treatment plans under the supervision of the Physical Therapist. Contributes to treatment development and progression, participates in patient care and program development, completes required documentation, and follows current clinical standards and company policies.
Role Summary:
Provides quality physical therapy treatment consistent with A.P.T.A. professional and ethical standards. Completes accurate documentation in accordance with continuity of care and reimbursement requirements. Coordinates with other disciplines to support total patient care. Participates in discharge planning and provides patient/family education. Maintains clinical expertise through continuing education. Promotes teamwork within the department and company. Delegates assignments and supervises support staff, students, and volunteers as appropriate. Represents the Physical Therapy Department with patients, families, visitors, and staff. Attends care conferences as requested. Communicates effectively with patients, families, department staff, company personnel, and leadership. Secures necessary equipment and supplies. Performs other duties as assigned.
Required Qualifications:
Successful completion of an accredited Physical Therapy Assistant program Licensure as required by the
State of Michigan Skills and Abilities:
Professional judgment and initiative in identifying problems and recommending solutions Flexibility and initiative in the work environment Ability to respond effectively to patient and family needs Ability to bend, position, transfer, and lift patients weighing 80-300+ pounds without restriction, using clinically recommended lifting equipment
