You will collaborate closely with licensed physical therapists and healthcare professionals to implement treatment plans, assist patients in achieving their mobility and functional goals, and ensure exceptional patient care across various settings including outpatient clinics, inpatient facilities, skilled nursing facilities, and home health environments. Your expertise will help improve patient outcomes, promote recovery, and enhance overall well-being through skilled therapeutic interventions. Duties Assist licensed physical therapists in developing and implementing personalized treatment plans tailored to each patient's needs. Perform therapeutic exercises, manual therapy techniques such as trigger point therapy, and other modalities to promote healing and mobility. Operate and maintain medical equipment used in physical therapy treatments, ensuring safety and proper functionality. Document clinical assessments, treatment sessions, progress notes, and discharge plans accurately following clinical documentation standards. Collaborate with multidisciplinary healthcare teams—including physicians, nurses, occupational therapists, and caregivers—to coordinate comprehensive patient care. Manage electronic health records (EHR) efficiently to ensure timely updates of patient information and compliance with medical documentation requirements. Support discharge planning processes by preparing patients for transition to home or other care settings and providing caregiver communication for ongoing care needs. Skills Strong knowledge of physiology, anatomy, orthopedics, geriatrics, pediatrics, and related medical terminology essential for effective therapy delivery. Proficiency in clinical assessment documentation and adherence to clinical documentation standards. Skilled in manual therapy techniques such as trigger point therapy and other physiotherapy interventions. Experience operating medical equipment used in physical therapy treatments with attention to safety protocols. Excellent communication skills for engaging with patients of all ages—including pediatric and geriatric populations—and collaborating with healthcare professionals. Familiarity with inpatient settings, skilled nursing facilities, outpatient clinics, home health environments, and acute care experience. Ability to develop comprehensive care plans that align with patient goals and discharge strategies while maintaining high standards of medical documentation.
