Their role includes medication management, patient education, and coordination of care while maintaining accurate electronic health records and promoting overall health. Responsibilities Perform thorough patient assessments and clinical diagnoses Develop and implement treatment and medication plans Manage patient medication and monitor therapeutic outcomes Educate patients on health conditions and preventive care Coordinate care among multidisciplinary healthcare teams Maintain accurate and up-to-date electronic health records Promote health and wellness through patient counseling and education Provide timely follow-up care and reassessments Collaborate effectively with healthcare professionals Preferred Qualifications Entry-level experience in practice PA license (MI) Expertise in patient assessment and clinical diagnosis Knowledge of pharmacology and medication management Proficiency with electronic health records (EHR) systems Strong patient education and communication skills Critical thinking and problem-solving abilities Ability to work collaboratively in a team environment
