Hours:

32-38 hours. Full Time 4 or 5 days per week. One evening till 7pm.

Duties and Responsibilities :

• Conducts physical exams, taking medical history, image interpretation and patient evaluation/care.
• Consults with physician as needed and refers to physicians for more complicated.
• Orders diagnostic tests and scans and prescribes medications.
• Performs and documents history and physical examination based on age and history on complex and chronically-ill patients
• Updates and records changes in health status
• Formulates the appropriate differential diagnoses based on the history, physical exam, and clinical findings
• Formulates a plan of care to address complex, acute and chronic health care needs
• Synthesizes data from a variety of sources to make clinical judgments and decisions about appropriate recommendations and treatments
• Educates patients/families about self-management of acute/chronic illness with sensitivity to the patient's learning ability and cultural/ethnic background
• Employs appropriate diagnostic and therapeutic interventions and regimens with attention to safety, cost, invasiveness, simplicity, acceptability, adherence, and efficacy
• Promotes collaboration among members of the multidisciplinary health care team to facilitate optimal care for all patients

Job Qualifications :

• Qualified candidates must be MN licensed or eligible for licensure as a Physician Assistant in the State of MN.
• 2-3 years of clinical experience preferred. Family Medicine Preceptor rotation required.
• Excellent written and verbal communication skills.
• Strong computer skills, experience with electronic health records.
• Ability to work independently and on a team.
• Ability to travel between three sites locally if needed.
