Job Description

Position Summary The Social Services Designee is responsible for promoting the psychosocial well-being of residents by providing compassionate support, coordinating services, and assisting residents and families throughout their stay. Working collaboratively with the interdisciplinary team, this role helps ensure residents receive person-centered care that supports their emotional, social, and discharge planning needs while maintaining compliance with federal, state, and facility regulations. Essential Duties and Responsibilities Complete psychosocial assessments for residents upon admission, quarterly, annually, and with significant changes in condition. Identify residents' emotional, social, cultural, and psychosocial needs and develop appropriate interventions. Participate in developing, implementing, and updating individualized care plans. Provide emotional support, counseling, and advocacy for residents and their families. Assist residents and families in adjusting to admission, changes in health status, and transitions of care. Coordinate and facilitate discharge planning to ensure safe, appropriate, and timely transitions from the facility. Collaborate with physicians, nursing staff, therapy, dietary, activities, and other interdisciplinary team members to address residents' needs. Serve as a resource for residents and families by providing education, referrals, and access to community resources. Assist with advance directives, resident rights education, and other psychosocial concerns as appropriate. Document assessments, interventions, care planning, and resident interactions accurately and in accordance with facility policy and regulatory requirements. Participate in interdisciplinary meetings, quality improvement activities, and care conferences. Monitor residents for psychosocial changes and communicate concerns promptly to the care team. Maintain confidentiality of resident information and uphold residents' dignity, rights, and choices. Qualifications Experience working in a skilled nursing facility or long-term care setting required. Knowledge of psychosocial assessment, resident rights, and discharge planning processes. Strong interpersonal, communication, and organizational skills. Ability to establish trusting relationships with residents, families, and interdisciplinary team members. Ability to prioritize multiple responsibilities while maintaining attention to detail. Proficiency with electronic health records and standard computer applications preferred. Physical Requirements Ability to walk throughout the facility and sit for extended periods. Ability to communicate effectively with residents, families, staff, and community partners. Ability to occasionally lift or move up to 20 pounds. Working Conditions Work is performed in a skilled nursing facility with frequent interaction with residents, families, staff, healthcare providers, and community agencies. The position requires flexibility to respond to resident needs in a compassionate, professional, and timely manner.
