Key Responsibilities:

Conduct in-home comprehensive health risk assessments for Medicare Advantage members Review patients previous and current medical histories, conditions, and medication regimens Offer education to patients on their health conditions and medication usage Identify gaps in care and recommend resolutions to support optimal health outcomes No treatment planning or medication prescribing required

Qualifications:

Valid Nurse Practitioner license in Illinois MSN degree from an accredited nursing school Active Board Certification (AANP or ANCC) Current CPR/BLS certification Minimum of 6 months Nurse Practitioner experience preferred
