Job Description

Make a meaningful impact by providing essential healthcare services directly to patients where they feel most comfortabletheir homes. This per diem opportunity involves conducting in-home health risk assessments for Medicare Advantage members across various communities within Cass County, including Logansport, Galveston, and Walton. Each assessment averages 45 minutes, offering flexibility and patient-centered care without the responsibilities of ongoing treatment or medication prescribing.
Schedule:
Flexible shifts available Monday through Sunday Freedom to create your own work schedule Choose between minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
Complete comprehensive annual health risk assessments for Medicare Advantage members Review and assess prior and current health conditions, medical histories, and medication usage Educate patients on their diagnoses, management strategies, and prescribed medications Identify gaps in care and provide actionable recommendations to improve patient outcomes Focus on assessment and educationno requirement for treatment plan creation or medication prescriptions
Qualifications:
Active and unrestricted Nurse Practitioner license in Indiana Valid CPR certification (BLS) National board certification (AANP or ANCC) Minimum of six months experience as a practicing Nurse Practitioner preferred Masters degree in Nursing (MSN) from an accredited institution
