Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members in Logansport, Galveston, Walton, and other areas within Cass County Review prior and current medical history, health conditions, and medication profiles Deliver patient education regarding medical diagnoses and prescribed therapies Identify care gaps and recommend appropriate follow-up actions to optimize patient outcomes No requirement to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in the state of Indiana Current CPR certification (BLS) National board certification through AANP or ANCC Minimum of six months of recent nurse practitioner experience is preferred MSN degree conferred by an accredited nursing program Enjoy earning $80-$90 per completed evaluation, with an additional mileage stipend and reimbursement for administrative time, reflecting the commitment to valuing your expertise and time.
