Description The Hospice RN Visit Nurse is responsible to plan, organize and direct home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Position:

RN Visit Nurse for

Hospice Hourly Range:

$40.00 - $45.00 hourly, depending on experience

Service Area:

Minneapolis/St. Paul and surrounding cities (plus mileage reimbursement of 72.5 cents per mile)

Schedule:

PRN/Casual/Part-Time. Minimum of 8 hours a week and one weekend shift per quarter (weekend and on-call rotation as needed, no holiday rotation) Lifespark is a complete senior health company headquartered in St. Louis Park, Minnesota. Position Functions & Responsibilities:

Completes an initial assessment of client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).

Regularly re-evaluates client nursing needs.

Initiates the plan of care and makes necessary revisions as client status and needs change.

Uses health assessment data to determine nursing diagnosis.

Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and palliative nursing actions. Includes the client and the family in the planning process.

Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State Nurse Practice Act.

Counsels the client and family in meeting nursing and related needs.

Provides health care instructions to the client as appropriate per assessment and plan of care.

Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.

Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

Lead by example in establishing rapport with clients, clinicians, partners, and the community.

Ensure positive/professional communication within the team

Customer service resolution; escalate concerns

Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.

Communicates with the physician regarding the client's needs and reports any changes in the client's condition; obtains/receives physician's orders as required.

Communicates with community health related persons to coordinate the care plan.

Timely completion of all documentation as it relates to the care plan by 10am the next day

Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse

Completes orientation and annual training requirements as assigned

Participates in Lifespark's Event Management process including initiating reports, escalation of events and investigations

Promotes safety in the workplace through real time coaching and participation in safety awareness training

Manages client supplies assuring appropriate DME provider is utilized.

Schedules and keeps member appointments

Organizes travel efficiently and documents mileage accurately within EMR

Other duties as assigned

Qualifications:
  • One to two years of recent acute care experience in an institutional setting. One year of home health care experience preferred.
  • Experience in customer service/direct member contact field preferred
  • Experience supervising caregivers
  • Graduate of an accredited school of nursing
  • Current Minnesota Nursing License
