Job Description

Deliver impactful patient care as a Nurse Practitioner conducting pivotal home health risk assessments for Medicare Advantage plan members across Boone County areas, including Columbia and its surrounding communities. This per diem opportunity offers the autonomy to craft your own schedule, with flexible shifts available any day of the week.
Schedule:
Flexible shifts, Monday through Sunday Option to create your own schedule Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
Perform comprehensive annual and post-discharge home health risk assessments (approximately 45 minutes each) Review and evaluate patients prior and current medical histories, existing conditions, and medication use Educate members on their health status, conditions, and medications to empower better self-care Identify care gaps and suggest recommendations for improvement to enhance patient outcomes No treatment planning or medication prescribing required
Qualifications:
Active Nurse Practitioner license in Missouri Current BLS (CPR) certification Board Certification via AANP or ANCC At least 6 months of current NP experience preferred MSN degree from an accredited nursing school This role provides pay of $80$90 for each evaluation completed, plus a mileage stipend and compensation for administrative time. You will play a critical role in supporting Medicare recipients by identifying health risks, providing crucial education, and making meaningful recommendationsall while maintaining flexibility and autonomy in your work schedule.
