Responsibilities:

Complete annual home health risk assessments for Medicare Advantage members in their homes Thoroughly review medical and medication history, prior and current medical conditions Provide educational guidance to patients regarding health conditions and prescribed medications Identify care gaps, delivering recommendations for improved health management No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Missouri Board certified by AANP or ANCC Current BLS (CPR) certification Master of Science in Nursing from an accredited program Minimum of 6 months recent experience as a Nurse Practitioner is preferred This role offers competitive compensation of $80$90 per evaluation, with additional mileage stipend and pay for administrative time.
