Key Responsibilities:

Conduct home visits to perform annual or post-discharge health risk assessments Review patients prior and current medical, surgical, and medication histories Carry out comprehensive evaluations in approximately 45-minute sessions Educate patients on their medical conditions and prescribed medications Identify and communicate gaps in care, offering recommendations to improve patient outcomes No treatment planning or medication prescribing is required

Qualifications:

Active Nurse Practitioner licensure in North Carolina Current BLS certification (CPR) Board certification from AANP or ANCC Minimum of six months current experience as a Nurse Practitioner (preferred) MSN degree from an accredited nursing program Enjoy competitive compensation of $80$90 per evaluation along with a mileage stipend and pay for administrative time.
