Responsibilities:

Perform annual health assessments and post-discharge visits in patient homes Review and evaluate previous and current medical history, conditions, and medications Educate patients on chronic disease management and safe medication use Identify care gaps and communicate practical recommendations for follow-up Focus on assessment onlyno prescribing or treatment plan development required

Qualifications:

Active Nurse Practitioner license in North Carolina Current CPR certification (BLS required) Board certification through AANP or ANCC At least six months of recent Nurse Practitioner experience is preferred MSN degree from an accredited nursing program As part of this per diem role, you will earn $80-$90 for each completed evaluation, plus a mileage stipend and administrative time.
