3002585005-2 Apply for this job now Job Description Registered Nurse - Rehab Del Sol Medical Center in El Paso, TX is seeking a compassionate Registered Nurse to join our Rehab team on a PRN basis. In this role, you will deliver direct, evidence-based patient care, coordinate treatment plans, and collaborate with an interdisciplinary team to support patients through rehabilitation and recovery. You will assess, monitor, and document patient status, educate patients and families, and advocate for quality outcomes. Responsibilities Provide direct nursing care to rehabilitation patients in an acute care hospital setting Perform comprehensive assessments and ongoing reassessments of patients' physical and functional status Develop, implement, and evaluate individualized nursing and rehabilitation care plans in collaboration with the interdisciplinary team Administer medications, treatments, and therapies safely and accurately following physician orders and hospital protocols Monitor, interpret, and document patient responses, vital signs, and changes in condition using the electronic medical record Reinforce and support physical, occupational, and speech therapy goals to promote mobility, independence, and functional recovery Educate patients and families on disease processes, medications, safety, and home care needs related to rehabilitation and discharge plans Implement and maintain fall prevention, skin integrity, infection control, and other safety measures Participate in interdisciplinary rounds, care conferences, and discharge planning to ensure coordinated patient care Respond promptly to emergencies and changes in patient status, using sound clinical judgment and escalation pathways Maintain accurate, timely documentation in accordance with hospital policies and regulatory standards Contribute to quality improvement, patient satisfaction, and evidence-based practice initiatives within the rehab unit Support and mentor new staff or students as appropriate, modeling professional behavior and compassionate care Required Skills Acute care nursing Rehabilitation nursing Patient assessment and reassessment Care planning and care coordination Medication administration and IV therapy Wound and skin integrity management Fall risk assessment and prevention Pain assessment and management Cardiac and hemodynamic monitoring basics Post-surgical and post-acute care Electronic medical record (EMR) documentation Patient and family education Infection prevention and control Basic life support and emergency response Interdisciplinary team collaboration Discharge planning support Time management and prioritization Clinical decision-making and critical thinking
