Job Description

This is a contract opportunity designed for Registered Nurses with experience in both long-term care and rehabilitation settings. Qualifications:
Valid RN license (current and unrestricted) Prior experience in long-term care and/or rehabilitation nursing Strong assessment, documentation, and clinical skills BLS/CPR certification Ability to communicate effectively with patients, families, and multidisciplinary teams Compassionate, patient-focused, and adaptable
Responsibilities:
Assess, plan, implement, and evaluate individualized patient care plans in collaboration with other healthcare professionals Administer medications, monitor treatment progress, and adjust interventions as necessary Provide wound care, IV therapy, and other skilled nursing services Maintain accurate and timely patient records in compliance with facility policies Educate patients and families about care plans, support resources, and preventive health measures Participate in interdisciplinary meetings and contribute to care planning Uphold infection control, safety, and quality improvement standards throughout all nursing practices
