Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Review and evaluate prior and current medical history, conditions, and medication usage Educate patients about their health conditions and medications Identify care gaps and provide recommendations to support patient well-being Visits may include annual assessments or post-discharge check-ins No responsibility for treatment plan creation or medication prescribing

Qualifications:

Active Nurse Practitioner license in the state of New York MSN degree from an accredited nursing school Current CPR (BLS) certification required Board certification through AANP or ANCC At least 6 months of current NP experience preferred

Compensation & Benefits:

This position offers $80$90 per evaluation, with additional mileage reimbursement and compensation for administrative time.
