Full Job Description Requirements:

Experienced Home Health RN Case Manager required Home Health experience required HCHB experience required Must be comfortable working independently in a field-based home health environment Territory covers the West Side of Portland with travel up to a 60-mile radius Productivity expectation: 25 points/week Day 1 orientation in office Company-issued tablets or phone provided Candidates open to occasional emergency staffing may be contacted

$55.00 - $65.00 per hour
