Flexible shifts available Monday through Sunday Self-directed schedulingchoose shifts that fit your lifestyle Minimum weekly or bi-weekly hourly commitment Responsibilities include: Conducting annual risk assessments in patients homes, approximately 45 minutes per evaluation Reviewing medical history, current conditions, and medication regimens for each patient Educating patients on their health status, chronic conditions, and medications Identifying potential gaps in care and providing actionable recommendations No requirement for developing treatment plans or prescribing medications

Desired Qualifications:

Active Nurse Practitioner license in South Carolina Board Certification through AANP or

ANCC MSN

degree from an accredited institution At least 6 months of recent NP experience (preferred) Current Basic Life Support (BLS/CPR) certification
