Job Description

Expand your clinical experience as a Nurse Practitioner specializing in home health visits in a per diem capacity. This role offers you the flexibility to create your own schedule, with the opportunity to serve a variety of Medicare Advantage plan members through home health risk assessments. Youll be traveling throughout areas such as Orangeburg, Holly Hill, and North in Orangeburg County, South Carolina, meeting the needs of community members in their homes and making a meaningful impact on their health outcomes.
Schedule:
Flexible shifts available Monday through Sunday Craft your own schedule based on your availability Minimum weekly or bi-weekly commitment
Key Responsibilities:
Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members in patients homes Review prior and present medical history, including current conditions and medication usage Provide patient education regarding diagnoses, treatment, and medications Identify care gaps and recommend interventions to address them Complete each risk assessment in approximately 45 minutes No requirement to create formal treatment plans or prescribe medications
Qualifications:
Active Nurse Practitioner license in South Carolina Board Certification through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited nursing program At least 6 months of current NP experience (preferred)
Compensation:
You will be compensated at $80$90 per evaluation. In addition, a mileage stipend and administrative time are included to support your work and travel.
