Schedule and Setting:

Monday Friday, 8:00 AM 5:00 PM Field-based visits to patient homes within assigned territory Caseload average: 35 patient visits per day (30 points per week) Daily travel required (mileage reimbursement provided)

Key Responsibilities:

Complete thorough OASIS assessments and develop individualized care plans Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management Continuously assess patient progress, report notable changes, and coordinate care with physicians and other clinicians Educate patients and their families about treatment plans, medications, disease management, and preventive health practices Ensure timely, accurate documentation in the Home Care Home Base (HCHB) system Coordinate closely with physical therapists, occupational therapists, social workers, and home health aides Engage in staff meetings, case conferences, and ongoing training Uphold all regulatory and agency standards for documentation and delivery of care

Qualifications:

Active RN license in good standing for the state of practice Minimum one year of recent experience in home health nursing (field experience highly preferred) Proficient with Home Care Home Base (HCHB) and OASIS Reliable vehicle, valid drivers license, and current auto insurance Benefits include weekly pay, access to comprehensive medical, dental, and vision insurance plans, a 401(k) retirement savings program, round-the-clock support, mileage reimbursement, and potential opportunities for contract extensions.
