Schedule and Setting:

Monday Friday, 8:00 AM 5:00 PM Field-based visits to patient homes within assigned territory Caseload average: 35 patient visits per day (30 points per week) Daily travel required (mileage reimbursement provided)

Key Responsibilities:

Complete thorough OASIS assessments and develop individualized care plans Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management Continuously assess patient progress, report notable changes, and coordinate care with physicians and other clinicians Educate patients and their families about treatment plans, medications, disease management, and preventive health practices Ensure timely, accurate documentation in the Home Care Home Base (HCHB) system Coordinate closely with physical therapists, occupational therapists, social workers, and home health aides Engage in staff meetings, case conferences, and ongoing training Uphold all regulatory and agency standards for documentation and delivery of care

Qualifications:
