Seeking an experienced Registered Nurse to deliver high-quality home health care in a dynamic, patient-focused field setting. This 13-week full-time contract role offers the opportunity to make a direct impact on individuals and families, supporting patients in the comfort of their homes.

Schedule & Work Environment:

Monday Friday, 8:00 AM 5:00 PM Field-based position involving daily travel to patient residences within the assigned territory Caseload expectation: 30 points per week (typically 35 patient visits per day)

Qualifications:

Active RN license in good standing in the state of practice At least 1 year of recent home health nursing experience (field experience preferred) Proficiency with electronic documentation using Home Care Home Base (HCHB) Demonstrated experience with OASIS assessments Reliable transportation, valid drivers license, and active auto insurance

Key Responsibilities:

Perform comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing interventions: wound care, medication administration, IV therapy, and chronic disease management Monitor patient progress, promptly identifying and communicating any changes to the interdisciplinary care team and physicians Educate patients and caregivers about disease management, treatment plans, and medication adherence Ensure accurate, timely, and regulatory-compliant documentation within HCHB Coordinate care with physical therapists, occupational therapists, social workers, and home health aides Participate in case conferences, staff meetings, and ongoing education to maintain quality standards
