Develop and implement personalized treatment plans, collaborate with medical teams, and educate patients and families. Monitor progress, document clinical notes, and complete required assessments and reports. Assist in patient evaluation, support functional communication, and promote safety and independence. Participate in team meetings and ensure compliance with agency protocols, including physician recertifications. Perform physical tasks such as bending, lifting, and standing for extended periods. This role offers a sign-on bonus, competitive salary ($90K-$125K), and benefits including health insurance, 401(k), paid time off, tuition reimbursement, and flexible scheduling (Monday-Friday, day shift). The position involves traveling to patients' homes and provides opportunities for professional growth and meaningful patient relationships.
