Mesa, AZ Glendale, AZ Phoenix , AZ Gilbert, AZ Yuma, AZ Tucson, AZ Queen Creek, AZ Maricopa, AZ Prescott, AZ Chandler, AZ Tolleson, AZ Surprise, AZ Responsibilities Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and personal hygiene. Monitor patients' health by observing vital signs, medication administration, and overall condition, ensuring safety and well-being. Support residents with mobility and transfer methods using equipment such as Hoyer lifts and patient transfer techniques. Prepare nutritious meals aligned with care plans and assist residents during meal times to promote proper nutrition. Maintain accurate documentation of care provided, including care plans, patient observations, and medication records, adhering to HIPAA regulations. Engage in patient observation to identify changes in behavior or health status, communicating findings promptly to healthcare professionals. Support residents' participation in community activities or outings, providing personal transportation assistance when needed. Follow infection control protocols through cleaning duties and maintaining a safe environment for residents and staff. Assist with specialized care needs such as dementia care, Alzheimer's care, memory care, hospice support, or working with individuals with autism or developmental disabilities. Requirements Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of patient monitoring techniques including vital signs measurement and medication administration. Ability to perform manual handling tasks and heavy lifting safely while supporting resident mobility. Certification in CPR and First Aid is required; additional certifications such as HIPAA compliance are advantageous. Experience working with individuals with neurodevelopmental disorders or disabilities is a plus. Strong communication skills for effective client interaction and team collaboration. Demonstrated ability to follow care plans meticulously while maintaining confidentiality and professionalism. Flexibility to work various shifts including evenings, weekends, or holidays as needed to ensure continuous resident support.
