Job Description

Overview We are seeking compassionate and dedicated Direct Care Workers to join our team immediately in Westchester, PA. In this vital role, you will provide personalized, respectful care to individuals with diverse needs, ensuring their safety, comfort, and dignity. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor patient observations closely, recording vital signs such as blood pressure, pulse, and temperature regularly. Administer medications accurately following medical documentation protocols and home healthcare regulations. Support clients with medical procedures like catheter care and assist with end-of-life or hospice care as needed. Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient information securely and accurately. Use patient transfer methods safely, including Hoyer lifts or other assistive devices, ensuring proper techniques to prevent injury. Maintain patient confidentiality in accordance with HIPAA standards while communicating effectively with clients and healthcare teams. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Follow all safety protocols by maintaining first aid and basic life support (CPR) certifications; respond promptly to emergencies. Support clients with mobility challenges through patient transfer methods and ensure their comfort during daily routines. Experience Previous assisted living or home healthcare experience preferred; familiarity with geriatric patients and dementia care is highly valued. Experience working with individuals with developmental disabilities or disabilities requiring specialized support. Knowledge of medical terminology, medication administration, and medical documentation practices is essential. Prior experience using EMR/EHR systems in a healthcare setting enhances your ability to succeed in this role. Demonstrated ability to perform heavy lifting safely and operate assistive devices such as Hoyer lifts or patient transfer equipment. Familiarity with home health agency operations, hospice care principles, and long-term care environments is advantageous. Certification in first aid, CPR, and basic life support is required; ongoing training is encouraged to stay current on best practices. Join us in providing compassionate care that transforms lives!
