•Saturday 7AM

•3PM (40 Hours) The Case Worker's primary duties include monitoring an assigned caseload of clients participating in a short-term life management program; developing program plans and goals; evaluating client progress through routine meetings with clients and/or staff; and advocating for clients by connecting them with services and resources that support their stability, independence, and ability to function within their environment.

Key Responsibilities:

Provides case management and support to clients in short-term life management and homeless outreach programs, including developing goals and monitoring progress. Conducts client intakes and assessments, coordinates shelter placement, and ensures clients understand program and shelter expectations. Maintains client records and documentation, including case notes, service records, and required information in the Client Services Information System (CSIS). Coordinates with community partners and service agencies to advocate for clients, provide referrals, communicate client progress, and connect clients with needed resources. Supports client stability and program operations by facilitating life-skills activities, monitoring shelter/program compliance, conducting inspections, and helping ensure program outcomes are met.

Physical Requirements and Working Conditions:

Knowledge of social service resources and agencies in the community. Knowledge of effective communication and motivation practices. Ability to develop program plans and goals based on client's needs. Ability to read, write, and communicate the English language. Ability to interview clients and to obtain pertinent information. Ability to prepare and maintain accurate and complete case notes and client records. Ability to build and maintain effective and professional working relationships with clients and community agencies. Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.) 5-10% of work time. Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking. Work is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.

EDUCATION AND EXPERIENCE

Two years of college coursework from an accredited college or university in Social Work, Behavioral Science, or a related field, and Two years experience working in a social or public service environment with experience assisting the public, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
